I think it's down to the referee's discretion to add on as much time as he thinks they will need to score a goal

I always think its more transparent  if they do nick it in injury time.  Cue goal delay, substitutions, goalies being arseholes at goal kicks (usually complete with booking) and questionable injuries all enhanced with overly dramatic refs pointing at watches.
